HHD 2019 Forms

We are busy preparing for the High Holy Days and are pleased to share with you that tickets, parking passes, and detailed service information will be mailed to you in the next few weeks. Our full schedule, childcare program and more information are on our website.

Below are links to memorialize a loved one in the Book of Life for Yom Kippur, to order a memorial plaque, and to order your Lulav and Etrog for Sukkot.

Thank you for choosing to spend your High Holy Days at Kol Shofar.

Memorial Plaques

It is an on-going High Holy Day tradition to place a permanent memorial plaque in the sanctuary with the name(s) of loved one(s). The plaque is lit on the person’s yahrzeit and on Yom Kippur, and the person’s name will be published every year in the Book of Life.

The cost is $500 per plaque.

Book of Life

At the High Holy Days, many congregants memorialize their loved ones of blessed memory in the Kol Shofar Book of Life. This year, we will have copies of the Book of Life available for everyone at our Yom Kippur services.

There are two ways to memorialize people this year: the Listing of name(s) can be printed for $36 each, or you can include a short written Tribute for $54 per loved one. Click here to purchase a memorial in the Book of Life

You have a choice between the Superior Set ($46 each) or a Standard Set ($36 each). Please complete this form and email it to Chloe or mail it to the synagogue immediately.
